- ID for all people in the household
- Proof of residency (utility bill, landlord statement, etc.)
- And any of the following will be accepted for proof of income (all income must be disclosed):
- DSS client ID cards
- 1040 forms
- Paycheck stub
- Medicaid or Food Stamp cards
- Pension retirement cards or statements
- WIC cards
- Unemployment Insurance paperwork
