Please list the names and death dates (exact date preferred, but month and year is acceptable) for the persons you wish us to search for in the spaces below. Use the reverse side of the form to list additional names and dates. Please note – very few obituaries were printed in the Jamestown newspaper before 1900.  Those that were printed are not the informative death notices that we know today. They were very brief and most did not mention surviving family members.

Mail the completed form, along with a check for $15.00 made payable to the James Prendergast Library to the address below. As our policy states, this fee covers one hour of research, copies and postage. You will be billed $7.50 for each additional hour of research required to find the names on your list.
